Physicals

Dear Students and Parents/Guardians,

Fall sports sign-ups start today Thurs. 5/9 through this google form link:  https://docs.google.com/forms/d/e/1FAIpQLSdjyfV9gZNYxXM93BhIETIBtNUVrKLVnCRsYcAtJ4hFJIrq0g/viewform.  Responses will be accepted through June 7 ^th.  That includes the following.

HS (current 8 ^th - 11 ^th graders):  Girls' Volleyball, Boys' & Girls' Cross Country, Girls' Tennis, Football, Boys' Soccer, Girls' Soccer, Field Hockey, Golf, Cheerleading, and Boys' & Girls' Water Polo (Muhlenberg Co-op)

MS (current 6 ^th & 7 ^th graders):  Girls' Volleyball, Boys' & Girls' Cross Country, Football, Boys' Soccer, Girls' Soccer, Field Hockey, Cheerleading

Also, there are physicals at Coca-Cola Park as an additional alternative.  You can find the dates and times below as well as the registration link for each of the dates listed. 

6/25 - 2pm to 6:45pm

7/16 - 2pm to 6:45pm

7/30 - 2pm to 5:45pm

Paperwork is attached.  Completed paperwork can either be emailed to  matdiehl@fleetwoodasd.org or mailed to the address below.  Email is preferred.  If you do not have access to the internet, please contact me at  matdiehl@fleetwoodasd.org to schedule a time to pick up hard copies of the forms.

First day of practice is Mon. 8/12 for HS (mandatory HS football heat acclimatization program and golf practice starts Mon. 8/5) and Mon. 8/19 for MS.
